Population Health services

The WACHS Great Southern Population Health team provides primary health care services across the region.  In partnership with a diverse range of agencies, organisations and consumers the team provides holistic health care programs for all ages.  These programs are extended to clients in the areas of child and maternal health, child development, school and youth health, Aboriginal health, allied health, public health and health promotion.

Services provided by Population Health within WACHS Great Southern include the following:

Aboriginal health

Great Southern Aboriginal Health provides free health care services to Aboriginal and Torres Strait Islander clients in the Lower and Central Great Southern Region.  Aboriginal Health Service clinics are located in Albany and Katanning.

The aim of Great Southern Aboriginal Health Service is to support our clients to achieve optimum health as best possible with a holistic approach, whilst we continue to work in partnership with other organisations. 

Key priority health areas are:

  • Child and family health, including antenatal care
  • Disease control including immunisations and sexual health
  • Chronic disease management for diabetes and asthma
  • Ear health and hearing
  • Social and emotional wellbeing and mental health
  • Hospital and prison liaison service
  • Transport services for health related appointments

Services delivered are provided to clients in a variety of ways:

  • in their home
  • at an Aboriginal Centre
  • at a community centre
  • at another health service as required
  • at our local clinics

Community child and school health services

Community child and school health services promote improved health outcomes for all children and their families across the region. The child and school health nurses deliver a wide range of primary health care services. The primary health care programs provided address child health, school and youth health, immunisation and health promotion.

These services are provided in child health clinics, schools, family’s homes and immunisation clinics across the region.

Primary health services

There are two primary health teams within Great Southern Population Health, one based in Albany and one in Katanning.

The Albany team provides child development services to the community of Albany and surrounds.

The Katanning team works to provide a broad range of allied health services to all age groups across the continuum of care, that is, inpatients, outpatients, rehabilitation and in the community setting including child development services.

Both primary health teams also provide regular outreach services to smaller sites across the region.

Public health and health promotion

The Public Health team, based in Albany, coordinates and manages communicable disease prevention and control, sexual health and blood borne virus, regional immunisation coordination, and pandemic disaster planning across the region. Support for infection control programs is also provided. The Public Health Unit facilitates and supports general practitioners, hospitals, community health teams, local government and other agencies in delivering public health services.

Health promotion is an integral part of all services provided by Population Health. Health promotion officers are based in Albany and Katanning and support the activities of other sectors within WACHS Great Southern. The health promotion officers have regional roles and work with government and non-government agencies and communities to support people and groups to improve their health. Health promotion officers provide a number of interventions, including resources, education, and training to individuals, groups, internal staff and external agencies.

Population Health service delivery models

Services are delivered to community members in community health centres, health clinics, hospitals, schools, homes and a variety of other community settings. Services are delivered using a variety of strategies to engage with individuals and groups. Where appropriate, multidisciplinary, trans-disciplinary and community development approaches are used.

Mental health services

WACHS Great Southern Mental Health Service (GSMHS) is a specialist service that provides acute mental health care for inpatient and community clients in the Great Southern region.   

The community teams are located in Albany and Katanning and consist of triage, adult, older adult, youth, child and adolescent teams, all staffed by mental health professionals. 

GSMHS employs Aboriginal mental health workers who assist the clinical team to provide culturally appropriate treatment.

Referrals are accepted from GPs and other health professionals. If urgent, clients may phone or self-present to Community Mental Health within office hours.

Seniors Health and Community Rehabilitation Services

The Great Southern Seniors Health and Community Rehabilitation team is dedicated to maintaining and improving the health independence and quality of life of older people, those with disabilities, their families and carers by ensuring high quality services are delivered in an equitable and timely manner.

Services provided by Seniors Health and Community Rehabilitation within WACHS Great Southern include the following:

Aged Care Assessment Team (ACAT)

The Aged Care Assessment Team (also known as the ACAT) helps older people and their carers work out what kind of care will best meet their needs when they are no longer able to manage at home without assistance. ACATs provide information on suitable care options and can help arrange referral to appropriate residential or community care.

Commonwealth funded care requiring an ACAT approval includes:

  • Residential Permanent - approvals that allow access to residential aged care
  • Residential Respite - approvals that allow access to residential respite care
  • Community - approvals to access a Home Care Package (HCP) or Transitional Care Package (TCP) or Short Term Restorative Care (STRC)

Community rehabilitation

The community rehabilitation service provides specialist rehabilitation to people over the age of 18 to help them live day to day life as actively and independently as possible. The team works with clients to maximise functional independence and quality of life.

The team offers rehabilitation therapies within the community setting throughout the Great Southern region.

Community Rehabilitation programs include:

  • Group rehabilitation and education clinics
    • Falls prevention clinics (Albany, Denmark, Mount Barker)
    • Parkinson’s disease programs
    • Neurological clinic – Stroke and other neurological conditions (Albany, Denmark, Mount Barker)
    • Pulmonary rehabilitation
  • Home visits / home exercise programs
  • Individual appointments

Community Rehabilitation team members include:

Older Patient Initiative (OPI)

The OPI team follows up older people after discharge from hospital if their risk screen has indicated a need for further care coordination to support their ongoing safety and independence in the community.
