PATS would like you to give you the opportunity to provide feedback by participating in a short confidential survey about your experience using PATS.  The information in this survey will be valuable in helping us review service improvements.  Space has been provided at the end of the survey for any additional comments you wish to provide.

 Please note this survey does not include feedback in relation to PATS eligibility or subsidy criteria; as this is aligned to Government Policy.

 Your answers are confidential and will not influence or impact the service you receive.
