Pilbara and Wheatbelt Combined Form

A combined PATS Application and Specialist Certification form is being used in the Pilbara and Wheatbelt regions only. The new single PATS form for you, your referring practitioner and medical specialist, has been distributed to GP’s throughout the Pilbara and Wheatbelt regions and is available in an alternative format upon request.  The form will be available from your Regional PATS Office, local hospital or you can download a copy of the form here

Process for PATS assistance before you travel     

  • Ask your GP to complete and sign Section B
  • Complete your information and sign Section A
  • Take/send your completed form to your local hospital or Regional PATS Office
  • Your form will be processed and you will be informed of the outcome by the Regional PATS Office
  • If approved take your form to your specialist appointment
  • Ask the specialist to complete and sign Section C
  • If your specialist is referring you onto a different speciality request Section C:1 be completed
  • Return your completed form to your local hospital/regional PATS office with any receipts within 8 weeks of your appointment.

  • Take your completed form to your specialist appointment
  • Ask the specialist to complete and sign Section C
  • If your specialist is referring you onto a different speciality request Section C:1 be completed
  • Return your completed form to your local hospital/Regional PATS office with any receipts within 8 weeks of your appointment.
  • Your application will be processed and either approved or declined.
