2025 Nursing and Midwifery Excellence Awards - Excellence in Person Centred Care – Team Award

The Midwest renal team has been named a finalist in the Excellence in Person-Centred Care – Team Award category at the 2025 WA Nursing and Midwifery Excellence Awards.

This well-deserved recognition highlights the team's dedication to providing exceptional, person-centred care to individuals living with kidney disease across the Midwest.

Through face-to-face consultations, home visits, telehealth and telephone support, the team makes high-quality renal care accessible, no matter where patients are located. They focus on early detection, prevention and management of chronic kidney disease, helping to improve long-term health outcomes.

The team also assist patients in accessing specialist services, pre-dialysis education, vascular access support and home therapies through establishing strong relationships with local health professionals and organisations to ensure patients receive optimal care close to home.

They serve more than 730 patients across 600,000 square kilometres and travelled over 10,000 km in 2023/24, providing 2,300 occasions of service. They also facilitated a visiting kidney transplant team, with six successful transplants to date.
